Jonathon Hope MBE
For over 30-years Jonathon has had kidney failure, experiencing virtually every step in the complex kidney patient pathway, including 14 years on dialysis. Since 2005 his fourth transplant has been successful.
Currently, Jonathon is co-chairing ‘Realising the Value of Individuals’ a project funded by NHS England designed to trial the Patient Activation Measure (PAM) and to assess the effect of innovative, supportive approaches to helping individuals in their own self care journey.
Over the last 10 years, Jonathon has held a number of leadership roles in healthcare improvement programmes nationally and across South East London. He is passionate about helping healthcare practitioners fully engage patients as genuine partners. His aims is for all patients to be empowered to be involved in their own care, where appropriate, and for their treatment plans to be aligned with their actual needs.
Previously Jonathon co-chaired the GSTT Charity £5mn End of Life Modernisation Initiative Community Work stream (2008-2011). He also chaired the Guy’s & St Thomas’ Charity (GSTT Charity) £5mn Kidney Modernisation Initiative (2003-2007) that helped radically redesign kidney services in Southeast London to meet patient needs.
Jonathon is also an inspirational public speaker and speaks regularly on a wide range of subjects, including whole system change, patient safety, patient involvement, leadership and governance, end of life care, kidney care, self-care and prevention.
In 2012 Jonathon was awarded an MBE for Services to Kidney Patients.
